- Care coordinator completes CFC-06 or Personal Care Agency representative completes PCA-08.
- Participant/legal representative and professional complete required original signatures and handwritten initials.
- Professional submits protected CFC materials in Harmony or PCS materials by secure DSM/fax as instructed.
