    • AFC liability release signed before flight departure by the applicable passengers and parent or legal guardian.

  3. Email your application and the document togetherTo request@angelflightcentral.org. Your social worker or doctor can send it instead if the document has to stay with them.

    • Submit the live AFC flight-request portal at least seven to ten business days before travel and provide appointment confirmation.
    • Have the physician return the signed medical release to request@angelflightcentral.org or fax 816-421-2409.

After you send

Expect a coordinator call within one to two business days; check voicemail and answer unfamiliar numbers. Confirm that AFC received the medical release and that commercial passes, Southwest terminals, and itinerary timing fit the request. If you have not heard back by then, write and ask.
