    What the form asks for

    They don’t publish the questions in advance. Have your contact details, your child’s diagnosis and date, and the hospital’s name to hand.

    • Use the official AFC flight-request portal and identify the trip as a compassion request, such as travel to a special-needs camp.
    • Provide complete requester, traveler, trip, contact, and equipment facts and any available destination-date verification.

After you send

Expect a coordinator call within one to two business days and confirm the trip is within AFC's case-by-case compassion scope. Keep ground transportation and a backup plan because AFC cannot guarantee a volunteer flight. If you have not heard back by then, write and ask.
