Before you start, check you fit

    • AFC liability release for applicable passengers and parent or legal guardian.

  3. Email your application and the document togetherTo request@angelflightcentral.org. Your social worker or doctor can send it instead if the document has to stay with them.

    • Submit the live AFC flight-request portal at least seven to ten business days before the flight date.
    • Provide appointment confirmation and have the physician return the signed medical release by email to request@angelflightcentral.org or fax to 816-421-2409.

After you send

Expect a coordinator call within one to two business days and confirm receipt of the medical release. Keep a backup plan and arrange ground transportation because pilot availability and weather can prevent a flight. If you have not heard back by then, write and ask.
