    Have ready

    • Adult or minor passenger liability release, selected for each passenger's age and signed before the flight.
    • Valid photo identification requested with the signed waiver and release.

  3. Submit it on their websiteSend the letter with your application, or the way they ask.

    • Submit through the official portal or call 800-549-9980.
    • Contact AFNE as soon as the medical appointment is confirmed and preferably at least five working days before travel.

After you send

Call the mission team if confirmation is not received or if appointment details change. Keep alternate transportation plans because volunteer flights may be delayed or cancelled.
