Before you start, check you fit

  • Your child is 17 or under.
  • Only immediate family members may stay unless the program approves an exception.
  • You must arrange and pay for transportation and most meals.

    What the form asks for

    They don’t publish the questions in advance. Have your contact details, your child’s diagnosis and date, and the hospital’s name to hand.

    Have ready

    • Completed Family Respite Form.
    • Court-ordered custody agreement when divorced or separated parents cannot agree to share program opportunities.

    • Submit the Family Respite Form and have the social worker or healthcare professional separately submit the Medical Respite Form.
    • Provide a custody order when the published divorced/separated-parent rule applies.
