    • Written legal-guardian consent for participation and the program's agreements.

    • Submit the live Youth Inquiry Form; a non-guardian referrer must provide the family's contact information so OAAT can obtain guardian consent and details.

After you send

Because selection is limited and review may take time, wait for OAAT to contact the guardian about verification, consent, interviews, and campaign next steps. If you have not heard back in two weeks, write and ask whether it arrived.
