Before you start, check you fit

    What the form asks for

    They don’t publish the questions in advance. Have your contact details, your child’s diagnosis and date, and the hospital’s name to hand.

    • Submit the fully completed official form and diagnosis-verification image through the referral portal after it reopens.

After you send

If the portal still says referrals are closed, do not submit elsewhere; recheck the official portal after the waiting list clears. If you have not heard back in two weeks, write and ask whether it arrived.
