- Current income documentation for all working household members covering the most recent 6-8 weeks, such as pay stubs, bank statements, benefit award letters, or tax returns as applicable.
- Government ID or utility bill showing the patient's name and address in an eligible county; a P.O. Box alone is insufficient.
- Most recent currently due and unpaid bill or notice for every requested expense, showing amount due, due date, account reference, and payee information.
What happens next
- Complete the secure portal application and upload the gathered documents.
- The provider or counselor submits the complete application package to The Cancer Foundation on your child’s behalf.
- Use the Client Portal to check status and request grant payments; contact the assistance team if the portal or document review stalls.
After your care team sends it
