What to bring to the meeting
- Your child’s diagnosis, the hospital’s name, and what you need help with.
What happens next
- The oncology medical team completes the current Emergency Relief Referral from the protected provider library.
- The oncology medical team faxes the completed Emergency Relief Referral to Cancer Support Services at 706-736-5142.
- Confirm receipt and ask whether any bill, prescription, supply documentation, or other protected-form attachment is still needed.
