    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • The medical social worker reviews the English or Spanish guidelines with the family and confirms the permanent-residence and temporary-stay rules.
    • The medical social worker completes every published referral field, including dates, family contacts, distance, bedroom preference, guest names, permanent-residence attestation, and the social worker signature.
    • The medical social worker submits the online referral, or sends the non-fillable referral PDF by the official email or fax route.
    • CCFSoCal evaluates medical need, other lodging options, distance traveled, and current space; the home operates on a wait-list basis with up to two families.

After your social worker sends it

The caretaker or CCFSoCal staff contacts an accepted family to arrange orientation and check-in. Because capacity is limited and wait-list based, ask the social worker to confirm receipt and current space.
