    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • The medical social worker completes all published family, referrer, and assistance-request fields.
    • The medical social worker submits the online form, or completes the non-fillable PDF and sends it by official email or fax.
    • CCFSoCal reviews eligibility and availability during the published 2-4 business-day assessment period.
    • CCFSoCal contacts the family directly if approved; the family follows the pickup or hospital-delivery instructions.

After your social worker sends it

Allow 2-4 business days for review. The family should wait for direct approval contact; the social worker may verify registration but pending assistance is not discussed before review.
