Before you start, check you fit

  • Your child is 18 or under.
  • Request each flight at least 14 days before departure.
  • Two tickets: your child and one authorized adult companion.

  1. Ask your social worker or doctor for the documentStart here. This is the part that takes the longest.

    Children's Flight of Hope needs 2 things from your social worker or doctor. It has to cover one thing. Copy this message, or say it in person at the next visit.

    What the form asks for

    They don’t publish the questions in advance. Have your contact details, your child’s diagnosis and date, and the hospital’s name to hand.

    Have ready

    • Virtual waiver of liability issued after the family receives a Client ID.

  3. Submit it on their websiteSend the document with your application, or the way they ask.

    • Submit the New Family Application first, then use the Client-ID-specific links for the financial form and waiver and deliver the provider-completed medical form as instructed.

After you send

Watch for the Missions Team email; after acceptance, use the approved-family portal for each flight request at least 14 days before departure. If you have not heard back by then, write and ask.
