- Your child’s diagnosis, the hospital’s name, and what you need help with.
What happens next
- Confirm with the primary cancer or BMT team when treatment ended and whether your child is ready for specialized survivor follow-up.
- The primary cancer or BMT healthcare team places the referral to the Cancer Survivor Program.
- Confirm that the referral was received and ask how the survivorship visit will be scheduled.

The family should confirm receipt and scheduling with the primary team or Cancer Survivor Program.
