    • Completed general honoree application supplied by CWFund after the public request form.
    • Completed general release agreement supplied in the CWFund packet.

    • First submit the public name-and-email form to request the packet.
    • Return the completed general application, medical eligibility form, and general release agreement using the instructions supplied with the packet.

After you send

Expect a confirmation email when the completed application is received. Applications are reviewed quarterly on a first-come, first-served basis; respond to any email identifying an incomplete part. If you have not heard back in two weeks, write and ask whether it arrived.
