Before you start, check you fit

  • The 2026 application deadline has passed; ask about next year's dates.
  • Every attendee signs a liability waiver and photo release.

  1. Ask your doctor for the documentStart here. This is the part that takes the longest.

    Dream Day on Cape Cod needs a signed confirmation of the diagnosis from your doctor. Copy this message, or say it in person at the next visit.

    • Liability waiver and media/photo release for every attendee.
