    Houston Children's Charity needs a letter from your doctor. It has to cover 2 things. Copy this message, or say it in person at the next visit.

We’d like to apply to Houston Children's Charity’s Chariots for Children. They offer chariots for Children awards wheelchair-accessible vans to us if our child uses a wheelchair, and we are checking whether they are taking requests.

They need a letter from you. It has to cover our child’s condition and why the child would benefit from a wheelchair-accessible van. Their form and instructions are here: https://houstonchildrenscharity.org/programs/chariots-children/

    What the form asks for

Do you have a Permanent Texas Driver's LicenseDate; name; spouse name; address; email; home, cell and work phone; rent/mortgage amount; landlord and phone; marital status; language; race/ethnicity; number of children in legal custody; wheelchair ownership; monthly household income including government assistanceEach child's name, age, gender, and birthdate

    Have ready

    • Handwritten letter explaining need for a wheelchair-accessible van and one paragraph about the child including personal facts, hobbies, and interests.
    • Each child's birth certificate, or legal guardianship paperwork if applicant is not the birth parent.
    • Permanent Texas driver's license; Limited Term licenses and passports are not accepted.
    • Rental agreement or mortgage document.
    • Most current monthly bills.
    • Food stamps, Medicaid, or other government-assistance letter.
    • Current pay stub.
    • Color photographs of the child;.

  3. Post your application and the letter togetherTo the address on the form.

    • Mail the completed original form and copies of every listed document to HCC at the address printed on the form.
    • Do not submit an incomplete packet or attempt walk-in delivery.

After you send

Call HCC if phone number or address changes. Because no review timeline is published, call to confirm receipt if needed. If you have not heard back in two weeks, write and ask whether it arrived.
