    • Confirm the child previously received JJCCF Financial Assistance, remains in critical financial need, and has exhausted other charitable help.
    • Obtain and complete the current one-page participation agreement with the hospital social worker.
    • Prepare the family financial-hardship and specific-needs facts for both required letters.
    • Social worker authors, reviews, and completes the admission-request letter; professional signature and credentials remain for the social worker.
