    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • Obtain the supplier or vendor invoice with patient name and cost when the request involves an invoiced expense.
    • Have a licensed or certified health care practitioner prepare the required written verification that other payment routes denied the requested service.
    • The referring social worker completes the application in full, including your child’s current condition and specific needs, using facts supplied by the family and care team.
    • The actual parent or guardian and referring social worker review and sign the accuracy attestation; the parent separately chooses whether to sign the optional photo-contact authorization.
