Before you start, check you fit

    Have ready

    • Most recent household federal tax-return page showing income, or benefits statement or recent pay stubs for all household members when no return is filed.
    • If requested during review: three months of household bank statements, employment verification, or state identification. Only if they ask.

  3. Post your application and the letter togetherTo the address on the form.

    • Submit securely through the direct ProposalCentral application with the clinician letter and current income evidence uploaded.
    • If online completion is impossible, request the mailed form and send the completed packet to the Patient Grant Program address published on the official page.

After you send

Online applications are generally reviewed within two weeks; mailed applications may take four to five weeks. Use ProposalCentral support at pcsupport@altum.com or 800-875-2562 only for login or technical problems. If you have not heard back by then, write and ask.
