Before you start, check you fit

  • Review may take two weeks; allow three to four weeks to receive a card.
  • Cards depend on available funds.
  • A provider or hospital social-worker letter is required.

After you send

Watch email and phone for receipt and approval notices; cards are sent weekly after approval, subject to available funds. If you have not heard back in two weeks, write and ask whether it arrived.
