- Completed and signed family Section 2 of the authorized MSA-0636 transportation invoice.
- Itemized, unaltered receipts when required for reimbursable travel, lodging, meals, tolls, fares, or parking.
After you send
MDHHS states reimbursement is issued within six to eight weeks after a complete invoice is received; follow up with the local CSHCS office about delays or incorrect authorization information. If you have not heard back by then, write and ask.
