- Completed parent/youth conference scholarship application, budget worksheet, and proposed budget.
- Conference agenda or brochure or website link; a draft agenda may be used if the official agenda is unavailable.
- Only the estimates and supporting documents for requested expenses, including the specified number of airfare, mileage, ground-transportation, hotel, parking, childcare, PDN, or accommodation estimates as applicable.
- W-9 and Conference Expense Form issued after conditional approval and due within 14 business days.
After you send
Allow up to 30 days for review. If conditionally approved, return the issued W-9 and expense form within 14 business days; after attending, return reimbursement documents and receipts within 14 business days. If you have not heard back by then, write and ask.
