- Completed and signed DCH-1426 only when the family does not use MI Bridges.
- Employer and income information for household members, such as pay stubs, W-2 forms, or wage and tax statements, as applicable.
- Policy numbers for current health insurance and information about job-related coverage available to the family, as applicable.
