    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • Complete all portal sections, including treatment confirmation, requested assistance, and the individual situation-of-need explanation.
    • Make the health attestation and obtain your child or representative authorization without delegating signatures or clinical opinions.
    • Upload all applicable income and expense documents and submit the completed online application.
    • Retain the emailed copy if requested and respond to requests for missing documentation.

After your social worker sends it

Keep the emailed copy if requested, watch for the decision notice, and respond promptly if the Foundation asks for more documentation.
