- Completed and signed MS-88 application/redetermination.
- Claims history, insurance card front/back, full Summary of Benefits, completed Insurance Verification form, and rate sheet.
- Most recent 30 days of paystubs showing deductions, or detailed self-pay bill and proof of payment; supporting contribution/fee documents as applicable.
After you send
Confirm all documentation arrived within 30 days of signature; after approval, submit premium proof within 60 days of each paid date. If you have not heard back by then, write and ask.
