- The student's Infinite Campus information for the initial assessment.
- Transcript, 504 plan, and/or IEP, when applicable.
After you send
Bring the requested school records to the initial assessment and watch for the Education Services Supervisor's follow-up call. If you have not heard back in two weeks, write and ask whether it arrived.
