- Your child’s diagnosis, the hospital’s name, and what you need help with.
What happens next
- The plan refers a potentially Medicaid-eligible child to C-YES, or a child who is not Medicaid eligible to a contracted Health Home entity for the HCBS eligibility assessment.
- The contracted Health Home entity completes the in-person HCBS eligibility assessment using plan-supplied worksheets and available provider documentation.
- If eligible, the Health Home entity sends the outcome to the plan and refers the child to approved HCBS providers; the family confirms the care-coordination owner.
After your care team sends it
The family should confirm who owns care coordination and which approved provider will deliver each authorized service.
