- Signed CMH release and consent, either on the MAF or the separate current form.
What happens next
- The CMH managing physician completes the provider, diagnosis, requested-services, examination-date, and diagnostic-plan portions and attaches the required medical material.
- The managing physician signs and submits the complete MAF, consent, and diagnostic plan to ODH so they are received within 60 days of the initial examination.
- Watch for ODH's written approval or proposed denial and answer any request for missing information through the managing physician or CMH contact.
After your care team sends it
ODH publishes that it will notify the family, major-service providers, local health department, and managing physician of approval or proposed denial within 60 days after the last necessary document is received.
