Before you start, check you fit

  • Your child is 21 or under.
  • The request must concern a future expense and must lack insurance coverage.
  • Only one request per person is accepted in a 12-month period, with no more than three requests in that person's lifetime.

    What the form asks for

    They don’t publish the questions in advance. Have your contact details, your child’s diagnosis and date, and the hospital’s name to hand.

    Have ready

    • First page of the most recent federal income tax return or W-2, or the published alternative income records if taxes were not filed.
    • Provider or equipment-vendor quote showing price, provider or vendor name and mailing address, and applicable discounts.
    • Insurance exclusion or denial letter on letterhead for the requested treatment or equipment.

  3. Submit it on their websiteSend the letter with your application, or the way they ask.

    • Submit through the official portal between the first and fifteenth of the month before the monthly cap is reached.

After you send

Respond if Oracle Health Foundation requests additional information. For status questions, use the published case-grants email and include the child's name and date of birth only in that direct secure correspondence. If you have not heard back in two weeks, write and ask whether it arrived.
