Before you start, check you fit

  • Flights depend on volunteer pilots; keep a backup travel plan.
  • A flight is not guaranteed because missions depend on volunteer-pilot and weather availability.

    What to bring to the meeting

    • Waiver of Liability signed by the patient and each companion, or by the appropriate parent or guardian for a minor.

    What happens next

    • An accepted professional or minister starts the flight request at least five working days before the requested date.
    • The requester completes and submits the current online mission request fields.
    • A qualifying professional completes both pages of the non-fillable Mission Request Form.
    • The treating physician completes and hand-signs the physician statement.

After your social worker sends it

Confirm that Pilots for Patients received every required document and that the five-working-day clock has started. Maintain backup transportation until a volunteer pilot is confirmed.
