Before you start, check you fit
- Your child is 18 or under.
- A family must wait 12 months after an award before applying again.
- Bills must be in the parent or guardian's name.

They don’t publish the questions in advance. Have your contact details, your child’s diagnosis and date, and the hospital’s name to hand.
Have ready
- Clear, complete, readable copy of the bill requested for payment.
- Landlord W-9 for a rent request.
After you send
If awarded a gift card, activate it by emailing giftcard@pinkyswear.org from the application email address. If you have not heard back in two weeks, write and ask whether it arrived.
