    What to bring to the meeting

    • Current photo ID for every person age 18 or older.
    • Liability Release and Housing Guidelines accepted during the full in-person application.

    What happens next

    • The TMC provider or social worker completes the referral and housing-specific questions.
    • Email the completed referral directly to jan@joyandhope.org.
    • Attend the PJH office appointment to complete the full application and associated forms.
