- Picture identification.
- Any proof that verifies Putnam County residence.
- Proof of income for the last 30 days, such as check stubs, a child-support printout, Social Security benefits letter, work-source registration, unemployment-benefits letter, or food-stamp verification.
- Breakdown of typical monthly expenses with copies of bills, if any.
