- Your child’s diagnosis and the date.
- The hospital’s name and your social worker’s name.
What they’ll send next
- Download the current annual Confirmation of Diagnosis form.
- Give the form to a qualified healthcare professional to confirm the blood-cancer diagnosis and Rochester treatment or evaluation frequency, then print their name, sign, and date it.
- Email the completed diagnosis confirmation to info@rdrmn.org or fax it to 800-219-6845, following any program-specific instructions received by email.
After you send
Ask Red Drop Resources to confirm receipt and identify any unpublished celebration-specific requirements. If you have not heard back in two weeks, write and ask whether it arrived.
