    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • Confirm your child is 22 or younger, has professionally confirmed cancer or a brain tumor, is in treatment, meets the New England residence-or-treatment route, and the family is at or below 350% of the federal poverty guideline.
    • Complete all applicable professional, patient, financial-use, contact, and payee sections in the official form.
    • Prepare the required eligibility attestation on official hospital or organization letterhead, leaving professional review, name, title, credentials, and any signature for the submitting professional.
    • Submit the completed Sophia Support Google Form using the authorized professional's email address.

After your social worker sends it

Monitor the submitting professional's contact channel for a response within 7 to 14 days. If no response arrives after 14 days, the authorized professional may email kimberly@sophiasfund.org with your child name and submission date; do not send additional protected health information unless requested through a secure route.
