What to bring to the meeting
- Completed Educational Support Parent Interest Form.
- Full financial application supplemental documents when the request is after the original approval year.
What happens next
- Download and begin the parent, child, teacher, requested-service, schedule, and request-reason fields on the official education form.
- Ask the social worker to review the request and author the required request letter.
- Determine whether the request is within one year of the original financial-assistance approval date.
- Within one year, submit the education form and social-worker letter; after one year, also complete the full financial application and supplements.
