Before you start, check you fit

  • A referral from a participating hospital social worker is required.
  • You need to show financial need.

    What to bring to the meeting

    • Your child’s diagnosis, the hospital’s name, and what you need help with.

    What happens next

    • Confirm that your child is being treated through a current Colorado referring agency and that the hospital social worker or care-team professional will make the referral.
    • Gather your child, parent or guardian, household, diagnosis, insurance, income, and doctor information listed by the portal before opening the single-session form.
    • Complete the required Patient stage, then proceed through Primary Contact, Other Contact, Family Members, Case Details, Referring Entity, and Assessment.
    • Review the completed seven-stage referral and submit it through the Colorado portal in the same session.

After your social worker sends it

Retain the submission confirmation and relay any follow-up request from There With Care to the family.
