Alabama program
Alabama paying your work-plan premium
Alabama HIPP helps with a work-plan premium when its cost review shows savings for Medicaid.
What it is
Alabama HIPP helps with a work-plan premium when its cost review shows savings for Medicaid.
Alabama HIPP can review whether helping pay an employer-plan premium would save Medicaid money. A child must have Medicaid, and the program checks the plan and premium before approving help.
Eligibility rules
- Your child must have Medicaid. Alabama decides whether the premium is cost-effective.
What you get
- The agency can pay an approved work-plan premium.
- Work insurance can remain alongside Medicaid.
What the help includes
- If approved, the state pays the premium for the people named in the approval letter, which can be your child alone or the whole family plan. Ask which before you count on it.
If you decide to apply
- Talk with the social worker about whether HIPP would help. If you want to pursue it, review the online or paper form at MyALHIPP together.
- Have ready: the employer premium amount, plan summary and your child’s Medicaid number.

After you ask
- MyALHIPP offers online and paper applications. Ask HIPP how to track your request, what else it needs and when to expect a decision. The program phone is 855-692-5447; fax is 855-357-1130. Paper correspondence goes to PO Box 240756, Montgomery, AL 36124.
Good to know
This is the agency’s cost comparison, not a test of whether the premium strains your budget.
Other details
- Have ready: the policyholder's Social Security number, the policy and group numbers, your child's Medicaid number, both sides of the insurance card, the benefit summary, and pay stubs showing the premium deduction.
Federal background: Medicaid premium assistance.
Official sources
“Could Medicaid help pay our work-plan premium, and what would accepting that help require? If this route is open, could you help us request the cost review?”
Why I’m asking: I want to know whether we can reduce the premium while keeping useful work coverage.

Who does what
The three parts, side by side. The agency decides; nobody on this page does.
You
Review the employer’s premium breakdown and HIPP application with the social worker.
Your social worker
The hospital's financial counsellor knows which families have had this answered before.
The care team
Nothing.
- Who decides
- The Alabama Medicaid Agency decides whether paying the premium is cost-effective.
- Ask the agency
- “My child has Medicaid and there is an employer plan with a premium. How do I ask the agency to pay that premium?”
How to apply
First step: Talk with the social worker about HIPP’s application at MyALHIPP or 855-692-5447.
- Get the employer's premium figure and the plan summary.
- Call 1-800-362-1504 and ask how a family asks the agency to pay the employer premium.
- Write down who you spoke to and what they said, because the route is not published.
Where it starts: The social worker can help review online or paper applications at MyALHIPP (https://www.myalhipp.com/), with the Medicaid number, insurance card, benefit summary, premium breakdown and payroll-deduction evidence.
What to gather
- The employer's monthly premium amount
- The plan summary
- The child's Medicaid number
How long: HIPP confirms the expected decision date and how to track the request.
What a yes looks like
The agency pays the premium and the work plan stays in force alongside Medicaid.
What a no looks like, and the next move
A written decision explains the cost review. The social worker can help compare remaining premium and coverage options.
Watch out
- HIPP confirms which premiums and family members an approval covers, the payment method and the expected decision date.
- The test is the agency's own cost comparison, not your budget.
Dates that change this
2026-09-10: Whether Alabama runs this as a working programme today, under the HIPP name or any other, could not be verified: the rule was readable but no current manual, form or phone line was. (not yet confirmed against the final rule)
The numbers and the rules
The arcane layer, kept on purpose. Checked September 11, 2026.
What it is worth
Alabama HIPP can help pay an approved employer-plan premium when its cost-effectiveness review supports it.
Covers: The work plan's premium paid by Medicaid when the cost test is met
Legal protection: Keeping a work plan running keeps its provider network available alongside Medicaid
What it costs the family: The written HIPP approval identifies covered premium costs and any remaining family share.
The eligibility facts, as published
- Income
- not a separate income test: the child must have Medicaid and the agency must find the premium cost-effective
- Residency
- Alabama
- Processing standard
- HIPP confirms the expected decision date and request status; no fixed approval date is promised.
Expect friction on: No current application, phone line or decision clock was found for this route
The trap: A cost-effective plan still needs HIPP approval. The approval identifies the eligible premium, covered people, payment method and ongoing proof required.
