Pay for approved care at home beyond what a child the same age would ordinarily need, for a child with Medi-Cal.
What it is
Pay for approved care at home beyond what a child the same age would ordinarily need, for a child with Medi-Cal.
IHSS can pay for personal care and certain medical tasks you do at home. The county compares your child’s needs with those of a child the same age. A cancer diagnosis alone does not establish paid hours.
Who can get it
- The county checks the applicable Medi-Cal route, assessed care needs and any Share of Cost. An ordinary child-income screen alone does not decide whether assessment or parent payment is available.
- A parent can be paid under some IHSS programs but not others; the county decides which one fits your child. Ask at the assessment whether a parent can be the paid worker.
- Protective supervision requires a qualifying mental or cognitive inability to self-direct. Nursing vigilance alone is different.
What you get
- Pay for approved hours actually worked, at the county wage; Los Angeles pays $19.64 an hour in 2026.
- Personal care, approved household tasks and clinician-ordered medical tasks.
- The county decides which tasks and how many hours are paid; the program maximum is not what a family gets.

If you decide to apply
- Ask the county IHSS office for an application and home assessment. Keep a note of each care task and how long it takes.
- Have ready: your child’s Medi-Cal number, a two-week task-and-minutes log and the oncologist’s orders for medical tasks. Ask about paid-worker enrollment forms if a parent hopes to be paid.

What happens next
- A completed written application has a published 30-day processing standard, with an exception for a pending disability decision.
- Services have a published 15-day arrangement period after approval. These are not deadlines for receiving a copy of a plan.
- The notice lists tasks and hours. Payment follows worker enrollment and only covers approved hours actually worked.
Good to know
Watching for fever or infection alone does not qualify as protective supervision. The county subtracts ordinary age-appropriate parenting.
What else to know
- The county can explain task-by-task reductions and the hearing deadline on its notice.
- When a parent earns IHSS wages for caring for their own child who receives SSI, those wages are not counted as parental income in that child’s SSI calculation.
- If a parent is paid through IHSS and lives with the child, that pay is usually free of income tax; county payroll explains how Social Security taxes and work credits are handled.

Who does what

How long: The county has 30 days from a completed written application to decide and mail the notice, longer when the medical certification is late. Ask for the assessment date in writing.
What a yes looks like

What a no looks like, and the next move
A notice with zero or few hours. Ask for the task-by-task reasons, correct any same-age comparison or missed paramedical task, and request a hearing by the printed date.
Watch out
- The county subtracts what any parent does for a child that age. Fever checks and infection watching alone are not “protective supervision”, which needs a mental or cognitive inability to self-direct.
- Parent as provider works under IPO, CFCO and IHSS-R since the 2024 change. The PCSP sub-program still bars parents. Ask which program your child's case sits in.
- Any applicable monthly Share of Cost must be resolved before payment. The county checks the service route and assessment; the income screen alone does not establish payable hours.
- IHSS pay to a parent for the SSI child does not count against the child's SSI. Tell Social Security it is IHSS.
- A provider who lives with the recipient may submit SOC 2298 for the qualifying IHSS or WPCS live-in income-tax exclusion. Social Security and Medicare payroll taxes follow separate family-employment rules: wages a child, as employer, pays a parent are generally excluded, subject to the federal family-employment exceptions. SOC 2298 does not itself create that payroll-tax exception. County payroll can confirm the recorded employer-provider relationship and explain whether these wages earn Social Security work credits.
- The published rule gives 30 days for processing a completed written application, with an exception for a pending disability decision, and 15 days after approval to arrange services. These are not a deadline for receiving a copy of the plan.
