What it is
A free care coordinator for children with special health needs through Connecticut’s Medical Home Initiative.
A coordinator can help keep referrals and support services together. Income does not block the coordination service. Small payments for particular needs have separate financial rules and funding limits.
Eligibility rules
- The program serves ages 0–21. Coordination has no income or insurance test.
- Gap payments are for uninsured or underinsured families.
- The coordinator confirms the financial rules and available funds for gap expenses, respite and camp. Free coordination has no income or insurance test. Respite and camp share a $500 annual family limit when funds are available.
