Treatment can make walking difficult, but the permit depends on a qualifying functional limitation. The clinician documents that limitation on Georgia form MV-9D. The issuing office decides whether the form meets its rules.
Eligibility rules
- The medical certification concerns mobility rather than diagnosis alone.
- A clinician must certify one of the functional conditions on MV-9D; cancer or a central line alone is not enough. For example, the form includes inability to walk 200 feet without resting or needing another person or an assistive device to walk. Georgia lists a licensed MD or DO, podiatrist, optometrist or chiropractor as certifiers; the parent or guardian completes the child’s application and submits it to the county tag office.
- Other MV-9D conditions include portable oxygen use, specified lung or cardiac impairment, and listed severe hearing or vision limitations. The certifier checks the form’s exact medical criteria rather than diagnosis alone.
