Treatment can make walking from the car to the clinic difficult. The permit depends on medical and walking limitations, not the diagnosis alone. A qualified clinician certifies those limitations on PA-3.

Eligibility rules
  • The permit depends on a qualifying mobility limitation, not leukemia alone. The current PA-3 form allows a physician, physician assistant or advanced practice registered nurse with the required Hawaii license or military authority to certify it.
  • PA-3 combines the listed medical and functional criteria. These can include inability to walk 200 feet without resting, needing another person or a device, or qualifying portable-oxygen use. Ordinary age-related inability to walk is not enough. The child holds the permit; a parent can act as representative. The form allows its specified notarized identity-affidavit alternative.
