Federal, exists in every state
Could medical bills open another Medicaid route?
Some states let qualifying medical bills help a family meet a Medicaid income test.
What it is
Some states let qualifying medical bills help a family meet a Medicaid income test.
Idaho has no spend-down route into Medicaid for a child; Idaho CHIP and Katie Beckett are the routes above the income line. There is nothing to apply for under this name here.
Eligibility rules
- Before treating spend-down as an option, your social worker can ask Idaho Medicaid whether a currently approved child coverage group allows it and request its written income, resource and medical-expense rules. A large bill or an adult disability income chart does not establish a child’s eligibility.
What you get
- A review of whether medical bills can affect Medicaid eligibility.
Federal background
- Where a spend-down exists, qualifying incurred bills can count without being paid first. The family remains responsible for the spend-down amount.
- The federal budget period is no longer than six months. Children covered as medically needy do not have the usual federal 12-month continuous-eligibility protection.
If you decide to apply
- Ask the Idaho Medicaid office whether children can qualify through a medically needy spend-down.
- If the office confirms a route, ask for its worksheet and bring bills, service dates and income records.

Good to know
Bills paid by another insurer do not count toward a spend-down. A large bill alone does not establish eligibility.
Official sources
“Does Idaho have a medical-bills route for children, and if so, what are its benefits and drawbacks compared with Katie Beckett? Could you help us decide whether to apply?”
Why I’m asking: We want to know whether medical bills change our child’s coverage options.

Who does what
The three parts, side by side. The agency decides; nobody on this page does.
You
Collect every unpaid bill with dates of service and ask for the worksheet in the month the bills are huge.
Your social worker
Confirms whether this state covers children as medically needy and what the standard is.
The care team
Records and letters when the application asks for them.
- Who decides
- The state Medicaid agency
- Ask the agency
- “Does this state offer Medicaid through medical bills for children?”
How to apply
First step: Ask the county or state Medicaid office for the medically needy worksheet only when unpaid bills in one month exceed your income minus the standard.
- Gather every unpaid bill with dates of service.
- Ask for the worksheet only in a month when bills owed exceed income minus the standard.
Where it starts: State or county medically needy worksheet
What to gather
- Every unpaid bill with dates of service
- Pay stubs for the budget month
- Bank balances on the first of the month if your state tests savings
How long: Judged per budget period (up to six months). Ask the office how it counts.
What a yes looks like
Medicaid for the covered month with the bills applied to the spend-down.
What a no looks like, and the next move
“Not enough expenses” or “no child program here”. Keep the bills for the hospital's financial assistance instead.
Watch out
- The standard is far below the ordinary Medicaid line. Most families need the disability options first.
- Bills a third party will pay do not count. Unpaid balances you owe do.
- A child covered this way has no 12-month continuous-eligibility protection.
The numbers and the rules
The arcane layer, kept on purpose. Checked September 7, 2026.
What it is worth
Medicaid for the budget period once the bills you owe reach your spend-down amount.
- Spend-down obligation — Spend-down obligation
Covers: State Medicaid services during the eligible period
Legal protection: Incurred (not paid) expenses count. Bills a third party will pay do not
What it costs the family: The family owes bills up to the spend-down amount.
The eligibility facts, as published
- State option required
- yes
- Medical expenses
- incurred, not paid
- Resource test
- often (Texas $2,000/$3,000; New Jersey $4,000/$6,000)
- Budget period
- no longer than 6 months
Decisions this site cannot make: Countable income and savings · Qualifying incurred expenses
Expect friction on: Very low standards · Recurring documentation
The trap: The standard is far below the ordinary Medicaid line, and a child covered this way has no 12-month continuous-eligibility protection.
What changes by state: Whether children are covered, the monthly standard, the resource limit, and the budget period (up to six months).
