Illinois program
Health coverage for a parent or relative caregiver (ACA Adult / FamilyCare Medicaid)
Illinois Medicaid can cover your own health care when your income meets the adult limit.
What it is
Illinois Medicaid can cover your own health care when your income meets the adult limit.
A drop in pay can open Medicaid for you during treatment, because Illinois judges adults on this month's income. A relative raising the child asks for their own coverage the same way.
Eligibility rules
- The ACA adult route covers ages 19 through 64 at an effective 138% of poverty.
- The 2026 monthly limits are $2,488 for two, $3,141 for three, $3,795 for four, $4,448 for five and $5,101 for six.
- DHS determines your Medicaid household and which income counts. An annual average may miss a recent drop in pay.
- FamilyCare includes caretaker relatives raising a child, so a grandparent or other relative can be covered on the adult line. DHS determines the adult category and household.
What you get
- Your own Medicaid coverage with no premium.
- An adult coverage review through the same ABE application used for your child.
What the help includes
- Adult coverage runs through a HealthChoice Illinois plan. Your doctors and pharmacy need to accept the selected plan.
If you decide to apply
- Ask the hospital financial counselor to include you in the ABE coverage application if you want a review.
- Have identification, current income records and your child’s ABE case number ready.

What happens next
- The application decision standard is 45 days. Approval provides your own coverage dates and plan choice.
Good to know
Your child can qualify even when you do not. The state uses a separate adult income test.
Other details
- From January 1, 2027, the state guidance describes work requirements for ACA adults. A parent, guardian, caretaker relative or family caregiver of a child under 14 or a person with a disability is exempt.
- The caseworker can identify the documents needed to establish a caregiving exemption.
Official sources
“Could the same application check coverage for me? What would change about my care, and could you help me apply if Medicaid would fit?”
Why I’m asking: I need to look after my own health while caring for my child, especially if my pay or work coverage changes.

Who does what
The three parts, side by side. The agency decides; nobody on this page does.
You
Tick yourself on the ABE application and report this month's income.
Your social worker
The social worker reminds you that the parent line is separate from the child's and helps report a pay drop.
The care team
Records and letters when the application asks for them.
- Who decides
- The Department of Human Services checks income for the whole application.
- Ask your social worker
- “Can the same ABE application check Medicaid for me, using this month's income?”
How to apply
First step: Add yourself to the ABE application at abe.illinois.gov.
- Apply for yourself on the same ABE application as your child, using this month's income.
Official application / program page ↗
Where it starts: Add yourself to the ABE application.
What to gather
- This month's income
- ID
- The child's ABE case number
How long: 45-day standard.
What a yes looks like
Your own Medicaid card and plan choice.
What a no looks like, and the next move
Over income: Get Covered Illinois within 60 days of losing a plan (90 days after Medicaid ends).
Watch out
- Medicaid counts this month's income: the month a paycheck stops is the month to apply.
- From January 2027, keep proof that you care for your child with a disability: that exempts you from the new work rules.
Dates that change this
2027-01-01: Adults 19-64 must meet work requirements from January 1, 2027; a parent or family caregiver of a child under 14 or of a person with a disability is exempt (HFS FAQ).
