Iowa’s adult Medicaid plan can cover you when your current income meets its rules.
A drop in work income can open coverage for you as well as your child. Iowa judges it on this month's income, on the lower adult line. You can add yourself to your child's application.
Eligibility rules
- The new community-engagement rules begin for new applicants December 1, 2026 and at renewals from February 2027.
- Published exclusions include caring for a child under 14 or a person with a disability.
What you get
- Hospital, medical and prescription coverage for a qualifying adult ages 19–64.
- No monthly contribution in the first year. Later contributions and some emergency-room charges can apply.
What the help includes
- IHAWP covers qualifying adults’ hospital, medical, prescription and other covered care. The first year has no monthly contribution. A copay may apply for nonemergency emergency-room use; the plan confirms that charge.
- There is no monthly contribution in the first year. From the second year it is $5 a month for incomes between 51% and 100% of poverty and $10 above that, and a yearly check-up plus a short health questionnaire waives it.
- Healthy Behaviors means a wellness or dental exam plus a health-risk assessment within 12 months, with a 30-day grace period. Medical exemptions, American Indian/Alaska Native rules and monthly hardship relief can matter. Hardship relief covers the current month, not automatically past or future months.
- At 51–100% of poverty, unpaid contributions do not end coverage but can leave a debt. At 101–133%, 90 days of nonpayment can end coverage, with re-enrollment possible. The counselor can check the notice and available exemptions.

- Ask the hospital’s financial counselor to include your own coverage request on the Iowa HHS application.
- Have your current income and insurance information ready, including any recent pay change.

- Your own coverage decision is separate from your child’s, even when both requests are on one form.
- A decision takes up to 45 days and coverage starts from the application month. From December 2026 new applicants must show 80 hours a month of work or similar activity, but a parent caring for a child under 14 is excused.

Other details
- A yearly income estimate can miss a recent drop in pay. The application uses the household and income Iowa counts for you.
- An adult relative caring for the child may qualify even if no parent lives in the home. Iowa checks that adult’s age, own Medicaid household and income; the child’s household screen does not decide it.

Watch out

- Iowa publishes the percentage but no monthly table, so the dollar figure here is worked out from the poverty line.
