Missouri program

Children’s insurance above the free line (CHIP)

Missouri’s child health insurance above the free income limit, sometimes with one premium for the family.

What it is

Missouri’s child health insurance above the free income limit, sometimes with one premium for the family.

CHIP uses the same MO HealthNet application and covers cancer treatment. Some families pay no premium. Above the free range, one monthly premium covers the family’s eligible children.

Eligibility rules
  • Your child must live in Missouri and be under 19. Enrollment in comprehensive insurance generally bars CHIP, with listed exceptions.
  • An employer offer bars only the premium groups, and only under Missouri’s affordability test. Coverage available through a state agency to a state employee is treated as affordable.
  • The April 2026–March 2027 monthly ceilings for four people are $5,088, $6,188 and $8,388 across the premium groups.
What you get
  • Some children have no premium. Premium coverage charges one monthly family amount based on age, income and household size.
  • Hospital care, chemotherapy, medicines and rides to covered appointments.
What the help includes
  • For a family of four the premium is $39 a month up to $5,088 of income, $127 up to $6,188, and $309 up to $8,388. One premium covers all the children.
  • Premium CHIP has separate payment and start-date rules, so earlier bills are not automatically covered. Missouri’s printed rule includes a highest-group delay and an exception for certain serious health needs. The financial counselor can help obtain the first-premium instructions and written start date, including an exception decision.
If you decide to apply
  1. Ask the hospital’s financial counselor to review the same MO HealthNet application at mydssapp.mo.gov.
  2. Include details of insurance your child has and any insurance offered through work. Ask what premium and start date would apply.

After you apply
  • Children receive 12 months of continuous eligibility. Missing a premium does not end coverage during that period.
Good to know

A work-plan offer differs from enrollment. Premium groups have Missouri’s affordability test, and infants have a higher free-coverage limit.

Other details
  • FSD distinguishes full health insurance from limited products such as accident-only or fixed-payment coverage. For premium CHIP, the employer-offer test uses an income-band formula based on a family-of-three poverty benchmark. It is not the Marketplace test or simply a percentage of your income. State-agency coverage has a special state-employee rule; other public employers are not automatically treated the same.
  • If work insurance for your child would cost you more than about $102, $168 or $256 a month (by income band), Missouri treats it as unaffordable and CHIP stays open to you.
Ask your social worker

“Would CHIP cover our child, and what would the family premium be? Could you explain the benefits and drawbacks and help us apply if it is a good fit?”

Why I’m asking: I want to compare the family premium with the treatment costs we could otherwise face.

Who does what

The three parts, side by side. The agency decides; nobody on this page does.

You

Apply once, and pay the first premium the day it is asked for.

Your social worker

The hospital financial counsellor can tell you which tier your income lands in before the notice arrives.

The care team

Nothing.

Ask your social worker
“Which CHIP tier would our income put us in, and what would the family premium be each month?”

How to apply

First step: Apply at mydssapp.mo.gov. One application is screened for the free side first and then for CHIP.

  1. Apply once at mydssapp.mo.gov; the same form is screened for the free side first.
  2. If a premium notice arrives, pay it immediately, because cover starts on that payment.
  3. If a parent works for a state agency or a school, ask what insurance is available to you before assuming CHIP is open.

Where it starts: The same MO HealthNet application at mydssapp.mo.gov. The financial counselor can help obtain written first-premium instructions, the start date and any serious-health-needs exception decision.

What to gather

  • This month's household income
  • Who lives at home
  • Any insurance card or written offer from an employer

How long: Missouri publishes no decision deadline for children's coverage.

What a yes looks like

A notice naming a tier and a monthly family premium, then a bill. Twelve months of continuous coverage follows the first payment.

What a no looks like, and the next move

If the no is about other insurance, ask which listed exception might apply. If it is about an employer offer, ask for the affordability test in writing.

Watch out

  • The premium is per family, not per child.
  • Cover starts when the first premium is paid, so pay it the day it is asked for.
  • An employer offer only blocks the premium tiers, and only if the state calls it affordable.

Dates that change this

2026-07-01: These premiums took effect on July 1, 2026 and the state says they may change each July 1. A public chart still dated July 2025 shows lower figures; the dated 2026 table is the one used here.

The numbers and the rules

The arcane layer, kept on purpose. Checked September 11, 2026.

What it is worth

Children's coverage to about $8,388 a month for four. No premium to 153% of poverty, then a monthly premium based on income group and household size.

  • $8,388/month — Top tier (CHIP 75) ceiling, family of 4, monthly
  • $5,088/month — First premium tier (CHIP 73) ceiling, family of 4, monthly
  • $6,188/month — Middle premium tier (CHIP 74) ceiling, family of 4, monthly
  • $39/month — First premium tier monthly premium, family of 4
  • $127/month — Middle premium tier monthly premium, family of 4
  • $309/month — Top premium tier monthly premium, family of 4

Covers: The same MO HealthNet benefit package as the free side · Chemotherapy, hospital care and prescriptions · Rides to appointments

Legal protection: Twelve months of continuous coverage; missing a premium is not a reason to close a child inside that period

What it costs the family: The state checks age-based free coverage first. Premium coverage uses one monthly family premium based on income group and counted household; for four people the July 1, 2026 amounts are $39, $127 or $309.

The eligibility facts, as published

Age
under 19
Age max exclusive
19
Income
MAGI household; tiers at 150%, 153%, 185%, 225% and about 305% of poverty
Insurance status condition
a child enrolled in comprehensive insurance is generally ineligible, with listed exceptions; an employer offer bars only the premium tiers, and only if affordable by the state's test
Residency
Missouri
Continuous eligibility
12 months
Statutory note
The highest premium group has a printed 30-day start rule and a serious-health-needs exception, separate from federally prohibited uninsured waiting periods. FSD confirms the operational start date and first-premium requirements.

The trap: An employer offer is not the same as being enrolled: an offer only bars the premium tiers, and only if it meets the state's affordability test. Insurance available to a state employee through a state agency counts as affordable whatever it costs, so a teacher or state worker should ask about this before assuming. Premium coverage starts when the first premium is paid, so pay it the day the bill arrives.
