What it is
Another way into MO HealthNet when your child meets disability rules and ordinary income limits are too low.
Spend-down compares your child’s budget with an individual income standard. The state calculates how much medical expense must be met each month. Some parental income and resources can still enter that calculation.
Eligibility rules
- The child route is for children under 18 who meet Social Security disability rules. From 18, parental income is no longer part of the disability applicant’s budget.
- Your child's own budget is compared with $1,131 a month; part of your income is counted as the child's. Savings in the child's name must be under about $6,220.
- Social Security counts childhood leukemia as a disability for at least 24 months after diagnosis or relapse (12 months after a transplant), so the disability test is usually met during treatment.
What you get
- Full MO HealthNet for months when the required medical-bill amount is met.
- Eligible bills can count before payment, if they remain your responsibility and another payer does not cover them.
What the help includes
- Coverage begins when the month’s spend-down is met. Paying the amount to the state is another option.
- The one-year deadline for submitting proof does not make every bill from the past year eligible. FSD checks the expense date, legal responsibility, other insurance and whether it was used before.

- Ask the hospital’s financial counselor about a MO HealthNet application with disability supplement IM-1ABDS.
- Have medical records and bills ready for the Family Support Division. Bills can go to FSD.IM.SPENDDOWNDOCS@DSS.MO.GOV.

After you apply
- FSD calculates the monthly amount and reviews the bills. Missouri has no general state SSI supplement for a disabled minor living at home. An SSI award does not automatically open MO HealthNet; coverage needs a separate review.
- The leukemia listing’s minimum period is not an automatic end to disability. Relapse, transplant and continuing disability need a separate assessment after that period.
Good to know
There is no family-of-four spend-down limit. The state must work out your child’s individual budget.

- Unpaid bills can count, including some from the three months before you apply. Bring every bill; the caseworker tells you which month each one counts toward.

First step: Apply at mydssapp.mo.gov and ask for the disabled-child route by name, with form IM-1ABDS.

- Ask the hospital billing office for an itemised statement each month and send it in.
- If an SSI decision is coming, do not wait for it: in Missouri that is a separate application.

How long: Missouri publishes no decision deadline. Once the amount is met and reported, coverage starts that day.

If the no is about disability, ask for the medical review's reasoning and appeal. If it is about resources, remember the limit is on the child's own money.

- There is no family-size figure here. Do not compare your whole household income with $1,131.
- A bill counts from the day you are charged, not the day you pay it.
- An SSI award does not open MO HealthNet in Missouri; it is a separate application.
Dates that change this
2026-09-10: Missouri is a 209(b) state: an SSI award does not open MO HealthNet on its own. Treat them as two applications.
2026-09-10: Whether Missouri has a Katie Beckett route could not be settled: the state plan attachment that would prove it was not obtainable. Ask the Family Support Division whether any category ignores parents' income for your child. (not yet confirmed against the final rule)
