Walking from the car can be difficult during treatment. A placard depends on what your child can do safely. Cancer alone is not the test.
Eligibility rules
- A physician, physician assistant, chiropractor or advanced practice registered nurse can certify.

- A free temporary disability parking placard.
What it covers
- A standard temporary placard lasts six months.
- An extended one can last up to 24 months.
If you decide to apply
- Ask the clinic whether your child’s limitations fit form MV5-DSB.
- If you choose to pursue a placard with your social worker, a physician, physician assistant, chiropractor or advanced practice registered nurse can review the May 2026 MV5 form and certify the qualifying condition.
