Heritage Health plans arrange rides through a transport service. It can help with hospital, specialist and pharmacy appointments. Your child’s plan determines the booking and longer-trip rules.
Eligibility rules
- Your child must be enrolled in Nebraska Medicaid. CHIP transportation eligibility needs a separate check.
What you get
- No-cost rides to and from covered appointments.
- Transportation for covered pharmacy and specialist visits.
What the help covers
- For Nebraska Total Care, trips over 30 miles in an urban county or 75 miles elsewhere need prior approval.
- Nebraska Total Care says ordinary gas reimbursement is not a covered Heritage Health benefit; a friend or relative would need an MTM contract to be paid as a driver. A medically needed attendant can be approved, and children under 19 ordinarily travel with an adult. Meals, lodging, additional escorts, out-of-state travel and any reimbursement need written plan or broker confirmation before the family relies on payment. Each plan confirms its own payment rates and allowances.

- Have the Medicaid card, plan name and appointment calendar ready, including pickup and destination details.

- The ride service confirms the pickup for each appointment. Routine booking lead times do not promise same-day transport.
- MTM booking numbers are Molina 888-889-0421, Nebraska Total Care 844-261-7834 and UnitedHealthcare 888-777-6924. Nebraska Total Care asks for at least two working days for routine trips; UnitedHealthcare’s 2026 manual says three calendar days. Molina’s routine lead time needs confirmation with its booking team. Urgent needs and hospital discharge need their own broker review; the ordinary lead time is not a promise of same-day transport.
Good to know
Nebraska Total Care requires advance approval for some longer trips. The mileage rule depends on where you live.

Dates that change this
2026-01-01: All three plans currently list MTM as the ride broker. An older state guidebook still names Modivcare for one plan; the current directory is used and the transition date was not published. (not yet confirmed against the final rule)
